Dosering vancomycine iv bij normale nierfunctie: 1000 mg 2 dd (<70kg) of 1000 mg 3 dd (>70kg) met een eerste vancomycine dalspiegel op dag 3.
Denk bij intraveneuze antibiotische therapie >3 dagen aan de mogelijkheid van thuisbehandeling (zie iPortal protocol: OPAT, antibiotica infuustherapie voor thuis).
